How to fill PAN form / application

A. If the applicant is an individual, a firm, company, body of individuals, association of persons etc;

  1. Apply for PAN in Form 49A, and fill the application form in block letters in English and preferably with black ink.
  2. If the applicant is a male then select shri in the application form, for married women smt, unmarried women kumari, and any other applicant other than individual such as a firm, company, trust etc should select M/S
  3. The applicant should provide his/her full name. Do not use the abbreviations in the first and last name, only one single character initial is allowed in the middle name for the individual applicant.

Example: Manohar Lal Dave (HUF)

  1. Individual applicants are allowed to provide abbreviated 'Name to be printed on the card. However the last name should be expanded. The abbreviated name should not be prefixed with any tittles.

       For example: 
       Last name/ Sur Name    First name           Middle Name
         THAKUR                        VISHAL                   SINGH
       The name can be written as:

  1. If the applicant is known by any other name(item no.3 in application form)   then it is mandatory to provide details of other name. No  need to submit any proofs, however it’s an optional field.
  2. Proof of identity and address should be submitted along with the application form
    1. Proof of Identity and Address should be in the name of the applicant as mentioned in item no. 1 of the application form.
    2. Document provided as Proof of Identity and Proof of Address should have name of the applicant exactly matching with the name in application.
      For Example: If name in application is mentioned as VISHLA SINGH THAKUR, proof of Identity and Address (except Ration card) should be in the name of VISHLA SINGH THAKUR in a single line.

    4. Applicants other than individuals/HUF/BOI, shall leave the residential address field blank.
  3. Telephone Number and e-mail ID

If telephone number/ mobile number is provided then STD/ISD/ country code should be mentioned.
If landline number then isd and std code,0091409********** where 0091 is country code and 40 is area code followed by the landline number.
Similarly where mobile number is mentioned then isd code only.

  1. Status of Applicant:  this is a mandatory field for all categories of applicant. Respective categories must be ticked e.g; company, individual, firm etc.
  2. Date of Birth / Incorporation / Agreement / Partnership or Trust Deed / Formation of Body of Individuals / Association of Persons

Relevant date for different category of applicants is:
Individual: Actual Date of Birth;
Company: Date of Incorporation;
Association of Person: Date of Formation/Creation;
Association of Persons(Trusts):Date of Creation of Trust Deed;
Partnership Firms: Date of Partnership Deed;
HUFs: date of Creation HUF and for ancestral HUF, date can be 01-01-0001 where the date of creation is not available.
Date cannot be a future date.

  1. Registration Number:

Companies should provide registration/CIN number as per the incorporation certificate.

  1. Salaried employee: The applicant has to mention the name of the organization.
  1. Photo:
    1. 'Individual' applicants should affix two recent colour photograph in the space provided in the application
    2. Please do not staple or clip the photograph
    3. Please ensure that the photograph has the clear visibility of applicant's face.
    4. An applicant other than individual like company, firm etc; need not provide any photograph.
    5. In the top left photo column of the application affix photo and sign across the photo.
    6. In the top right photo column of the application affix photo and do not sign on photo instead sign with black pen below the photo in the box provided.
  2. Signature /thumb impression:
    1. Signature/Left Thumb Impression should be put within the box provided in the application and prefer the black ink. Signature should not be put across or on the photograph.
    2. Individual applicants
    1. Non-Individual applicants
    1. Thumb Impression should be attested by a Magistrate or a Notary Public or a Gazetted Officer, under official seal and stamp.


B. If the applicant is a Minor



C. If the applicant is a HUF
